A clinical case is used to show the generally clinical, but more particularly the psychopathological and neurological symptoms of encephalopathia Wernicke. The main clinical manifestations of this syndrome are psychic alterations (Korsakow syndrome), disturbances in the gait, peripheral neuropathies and disturbances of the eye muscles. Neuropathological changes begin in the acute phase of the tissue damage, with permeability disturbance of the blood vessels and oedematous slackening of the brain substance, and disappearance of the myelin sheaths and cell processes, whilst the nerve cells - mostly without being involved - can show ischemic changes. This most frequently affects the diencephalon. Other areas of predilection are the hypothalamus, with the Corpora mamillaria and the thalamus with medio-dorsal nucleus, the dorsal nucleus of the vagus, the vestibular field in the Medulla oblongata, the caudal corpora quadrigemina region, and the central grey matter with the eye muscle nuclei in the mesencephalon. At the cicatricial stage the Wernicke focus is characterized by fibre being stored up in the walls of the vessels. In addition there are numerous large fibrillous astrocytes. The irreversible clinical deficiency symptoms can be attributed to these morphological findings at the chronic stage of Wernicke's affection.
