Charlie works as an assistant in the practice of Dr. Pain. They used too much laughing gas on a patient with terrible pains, so the doctor is searching a remedy against it. Charlie has to look after the patient during that time. To tranquillize the laughing patient, Charlie knocks him down with a wooden hammer. As the doctor comes back to the examination room, he's pushed out backwards.
Charlie causes some more trouble like slap other patients in the waiting room on their cheeks and begins to flirt with a lady.
